The diagram is in the engine manual.

What usually occurs is the o-ring fails to seat properly. There is a specified three step torque sequence that is necessary to properly seat the o-ring. This tells me that it's not a good design and can be a bit problematic in getting it back into position. Many folks have suffered a similar fate.
