I have just replaced all ignition coils, plugs and wires.
What could this be?

I had that on my 400E, and finally traced it to bad carbon tracking on one of the distributor caps. Digging a bit further, I found that the plug gaps were way too high, which increases the voltage drop inside the cap, which caused the carbon tracking. (They were the most expensive distributor caps I ever replaced!)
