The Italian tuneup usually refers to a high-speed blast down the freeway, getting the engine up to operating temperature and rushing lots of fuel and air through it, unclogging whatever might be in the way, burning off carbon, evaporating any moisture in the crankcase, etc. Free revving your engine with no load applied is not a good idea.
