I agree. Perhaps it did and just didn't tell OP?

"and couldn't shift it forward" is woefully lacking in detail.

Could he not get the lever to go into D or any of the subsequent forward selector positions?

Did he get it into gear but applying the pedal had no effect?

This is a 722.400 series transmission. The B2 failure was more common on the 300 series but I suppose it does not hurt to pop the cover off and look. Although I am not sure how serviceable the transmission is while inside the car, especially on a 201 chassis.

I would also check governor operating pressure and see what that is doing. I've never measured it but I know the governor is key in changing gears on the transmission.

I would also have the indy drop the AT pan...see if anything looks out of sorts. Metal in the fluid? Really dirty fluid?
