There is no negative feedback loop. Feedback is driven by the comsumers' out of pocket expenses. If the expense is low then there is no reason NOT to take advantage of everything the system has to offer, reasonable or not. For example, preventive care would be more important to individuals if they paid a greater proportion of medical care.
