So when I become a "Moderator" of the group, do I have to approve members?

Maybe. This depends on the "powers" given to you by an "Administrator", or some such title.
You may be given the ability to edit, delete, or "chastize". Again, it depends.

Do "public" groups get flooded out by clowns selling UGG* boots?

Sometimes. This is where Moderators earn their keep.
Been there. Done that. Have the T-shirt.
EDIT: Since you started the group and are the Mod, you can do whatever you want. Power, much?
Suggestion: If there is another person in the group whose judgement you trust, and will be around often, make them a Mod as well. Otherwise, if the group becomes popular with a lot of traffic, it can be a full time hobby.
