If your tranny fluid is ok and has been changed on schedule then....
In the engine bay look for the end of the throttle cable. You should see it on the left side of the engine (left side from the drivers chair that is) just under the air cleaner. You should see a white plastic wheel with a black plastic pointer. I need to take a picture to really show you. But you need to adjust the pointer until the black arrow points to the groove in the middle of the wheel. Your shift points should change.

The cold start may be a problem with your cold start injector but I would check the spark plugs and wires and dist cap for age and run a bottle of injector cleaner first.
