Tensioner and upper guide can be replaced pretty easily and cheaply, once done I wouldn't worry too much about the chain but it's so cheap ($70) I'd go ahead and do it.

My M116 in the 420 took a crap because all THREE upper guides broke, caused excessive slop in the chain, then the drivers side allowed for a skip of a cam tooth, bent all the valves in the head. Oh well, it'll have a firebreathing M117 stuffed in there.

That would be a 606. They have a tough chain setup, probably better then most of the older designs. Somehow I think there is more to this story, I have never heard of a 606 snapping a chain. Heck I wouldn't even consider changing one until it passed 300k.

I worked on two of my MB's with broken timing chains.

One that I bought that way. The chain broke because the vacuum pump failed. It bent some valves, broke some cam towers, and broke the cam. The chain wrapped itself around the crank sprocket. It was quite an effort to get that chain out. I had to remove the oil pump and the front main crank bearing cap. It was also bunched up in one of the chain guides. It broke off a piece of the end in pulling out the chain. After some consideration and thought, I left that guide as it was. This engine had just under 100k miles on it.

The other was a 190E 2.6 that broke while I was driving it. I was cruising down the freeway when it was as if the ignition was turned off. I rebuilt the head and replaced the chain and oil pump. The engine had ~200k miles on it.
