Only the way the outer pistons are connected to the crank....

Check out the Rootes TS3 Truck engine of the mid 60's.....

120% give or take per CC and with increased brake you get increased torque.

The Fairbanks-Morse diesel used in submarines and diesel locomotives were opposed piston engines also.
