I just replaced my tensioner and shock, the old one, probably original, was developing a bearing issue and the shock bushings were shot. Additionally, who ever removed the tensioner previously failed to reinstall the spacing washer that helps to move the pulley out to where it needs to be.

I also used locktite to help keep it where it's supposed to be.

I blame the missing spacing washer and poorly reinstalled tensioner for my 'episode' on the freeway last year where the serpentine belt slid off the end of the pulley, creating a lot of hate and discontent.
