Call me an agnostic on this one. Since there is no explanation of how it works, I have to guess.

Maybe higher than spec fuel pressure would produce better combustion in a fuel injected gasoline engine.

If they are using it on non-turbo diesels, maybe putting air into fuel line helps out. Back when, I had a GMC 6.2 diesel pickup with dual tanks, when the first tank was running out and it was sucking air into the fuel line, the truck ran like a bat out of Hell. Sort of like free turbo. Maybe that is the secret on the diesel claim.

Put up or shut up on this claim: Take two identical vehicles new from the factory, put the gizmo on one. Fill them both with same amount of fuel. Run them side by side on a test track swapping inside position each lap. See how much farther the car with the gizmo runs.

When I see the results of that, I will know what to believe.

Another way to stretch fuel consumption is to drive less.

I work at home at least 1 day per week. I save 20% on my commute fuel bill.

AMAZING, eh?

Its probably just a fuel heater or something simple like that. Just that alone might make the fuel less viscous enough to spray into a finer mist. That's probably part of why later MBs have them.
