I don't know about the smoke but it's possible that chunks of disintegrated head gasket are lodged in the lifter oil journals causing valve train noise and poor running. In fact if you find a bad head gasket, remove the plug at the back of the head and blow the lifter journals clean. Guess what? That means the cam and lifters have to come off the head.

If I let the chain fall into the abyss, wont I lose time? Is there a way to leave the cam gear in place? In other words, I was thinking of removing the cam bolt and leaving the gear with the chain just protruding from the block deck. Is that possible?

You won't lose time if you don't turn the engine while the head's off. The cam sprocket has to come off the chain to remove the upper guide. The upper guide has to come off because it traps the chain against the head. Once the upper guide is out you can get the sprocket back on the chain and lift the head around it. But why? The sprocket will have to come off the chain again when it goes back together.

But maybe you're more dexterous and can wiggle the upper guide with the cam sprocket zip tied to the chain.
