The knob does NOT come off the chrome stem. There is a lock nut at the bottom where it threads into the mechansim.

Yep, access to that nut is no pleasure without a suitable crow's foot.

It's not a hard job. I did it on my 124 about 10 years ago, so I don't remember offhand. I think if you do a forum search you'll find a thread. The console wood is easy enough to remove. Pull the ashtray, remove the 2 screws that hold the drawer unit in, pull the coin tray out, slide the wood panel forward and that part is done.

95 is a split year for 140 shift knobs. 92-early 95 are integral with shift lever, late 95-99 are bayonet mounted to the shift lever. Performance Products sells or used to sell an adapter shift lever to allow bayonet mount knobs on 124s. Should work for a 123 or 126 as well.

Maybe it doesn't fit the 123 and 126 - Mercedes® Shift Knob Adapter, 107/124 Chassis

Exactly, since that adapter does not work for a w123...
I grabbed a nice wood shift knob from mid 80s SD? or SDL? and installed on w123 300D;
The chrome stem was a different length and had unique bends to it, now my automatic gear points are slightly off (like Reverse being at neutral, etc)

IS the best solution to adjust neutral position at the trans?

Not stealing the thread, I think this is related
