It is simlar to a bad Injector and a bad Injector is often what causes the Prechamber issue.

An Injector that prays to the side can cut right though the sides of the Prechamber or cut a Glow Plug tip off.
If something falls to the bottom of the Prechamber and blocks enough holes the tip of the Prechamber gets blown off and when the Piston comes up it trys to squash the tip between the Piston and Cylinder Heads.

Not so catastrophic is that the Ball Pin can loosen in uses. Simply remove the Injector and make something with a Hook to go down inside and see if there is any Ball Pin movement.

You could also see our Tool Rental Program and see if anyone has the Tools to Pull the Chamber for rent.

When You pull the Chamber it peels Carbon off of it. Best to have the Piston up at TDC.
When I did that I found a Shop Vac easily vacuumed out the Carbon. I don't remember if I stuck the Vacuum Hose over the Injector Hole or the Glow Plug Hole but the resulting Air Flow pulled the crud out.
