Slight Knock at around 2200rpm

I've been getting a slight knock at around 22-2500 rpm when cold. Is this the result of bad fuel? Or is it more? Thanks! Oh, and the valves have been adjusted in recent memory, around 4k ago... Thanks!

Which Car? My 99 e300 had a "Knock Sound" at 1500 rpm, but it came from the exhaust. Replaced Catalyst and it runs great now.
