The above link is a new file. This is my 350SD within the last 100 miles. At idle. With 3 significant holes/fissures I've counted thusfar, at the top of piston #1. Each originating from against the cylinder wall. http://www.zippyvideos.com/2773151204709756/350toprun/ The one above this text here, is an older one with a higher quality picture, and starts from around 15 mph and ends near 115 mph. Also of a 350. But a 350SDL. Might as well post them here.

The injectors are less than a year old, cam/injection timing is dead on, valve lash is perfect, turbo seals are good, prechambers are carbon free, piston crowns are undamaged (Viewed through bore-scope), intake is crud/EGR-free, blow-by is so-so, oil pressure is great, and cold compression is 340-385 on all 5 (375-420 when at 82*c). I have to keep it between 850 and 1000rpm for a good idle (Partly due to the light 240D flywheel). Even when it's warm it will eventually start to smoke and randomly misfire if left at idle for long. B20 helps, but does not cure it. It runs like a crazy under power, it's just that embarrassing idle. |

That 350SD has a dead cylinder, at least thats what it sounds and looks like.
That 617 sounds pretty bad at idle. Strange seems like you have covered almost everything. Do delivery valves seals by any chance? Also the fact that the exhuast has a pretty major leak will affect how it sounds.

That IP must be different then the 603's that I am used to. On my engine delivery valve seals are nothing but an O ring, and copper crush washer. The parts cost a few bucks at most.
Rough idle is often a symtom of leaking seals, sometimes fuel is visable at the top of the IP. I would bet that this is an IP issue, you have gone over everything else.
