My '94 E320 I-6 has two three-into-one exhaust manifolds. Has anyone installed headers in replacement of the stock manifolds? Any performance gain? What brands are available? Where?

Don't assume that there is anything wrong with the cast manifolds. I owned a full house Brabus '84 300E and it had cast exhaust manifolds. AMG 300Es of the 80s all had cast manifolds. You can get steel tube headers, but with a cat and muffled exhaust on an engine close to stock you're not likely to see much performance boost. There are other, smarter, cheaper ways to gain a perfomance boost, such as rear end gears. My Brabus had gears, a built automatic trans, and a built engine. The rear gears came from a station wagon and are, by far, the cheapest way to improve acceleration.
