Macs are simple. Run software update every month and thats it. The operating system does all the upkeep and "defragging" automatically in the background during idle periods.

I use Powerbooks for years, a friend of mine is using a Powerbook I bought six years ago and it's still usable for every day work; Email, Word, browsing even light Photoshop work or editing running the latest Mac OS!

It's not 100% necessary, but it helps.

It's like Diesel Purge; I saw sluggish Mac's come back to live again with it. Repair Disk Permissions with with the Disk Utility will do the same off course.

Is it possible to install and run a Mac OS on a regular 'DOS' PC? I have a spare harddrive on my XP-desktop. Or does Mac require specific hardware config?

No. The OS does check for Cupertino DNA.

7 years and counting on my first G3 iMac. 5 additional computers since then and still not a single problem.
BTW that G3 iMac is still being used. To answer the original question, YES IT'S WORTH IT!

No. The mac hardware has alot of specific equipment that the MacOS needs to run. EFI instead of a BIOS, for one.
