Money, power, and the need for good PR. The shuttle program was falling behind its schedule, one more delay was not acceptable.

This represents the MO for most businesses. One or two people who have the necessary credentials and knowledge concerning a particular subject try and tell management they are making a HUGE mistake. Management refuses to listen and decides that "they know best". Idiots, all.

Obviously a major malfunction

When push came to shove, nobody at Thiokol, including the engineers, jumped ship and responded to NASA's question if anyone disagreed!
