I'm going with the Stalin thing. It is way less realistic, but when you are just making stuff up then who cares about reality!

Rs are schizo about Medicare. They cry endlessly about reducing entitlements and then when ideas are presented on reining in wildly implausible and uber-expensive end of life delay procedures, they revert to covering the ass with the electorate mode and cry "DEATH PANELS!"
