I always hold the pedal to the floor when it's 10 degrees or below. Starts every time.

My 240D without block heater started just fine after sitting overnight in -5F cold. I'd check valves, too. Also, 23-24 MPG is normal for an automatic, especially on winter diesel. 67BHP pulling...
