auto-transmission jerks in and out of gear

The automatic transmission in my 89 190e 2.6 jerks in and out of gear especially when going up hill from a start. Once the car has run for a while the problem seems to go away. The car has about 250k and otherwise works great. Could this be my torque converter going out? transmission? or just an adjustment?

It is most likely time for a rebuild. It sounds as if the clutches are slipping.
