First things first. Is it a W111 or a W108? A '69 280SE should be a W108

Huh. I put in the title a w111. I must be misunderstanding you ate you under the impression that in 69 there wasn't any 280se. I believe 280se w111 where made from 68-71. Not trying to disagree just for accuracy for others who may read this thread someday. Btw mine is a 69 w111 280se coupe.
Thanks

If it is a 4 door, it's a W108

When I lost track of the heater valve orientation while rebuilding the heater core in my 110, I set it by blowing through the valve until it was obvious where it was full flow, then mounting the valve arm at full extension of the cable.

Or you can try setting your heater cable to full heat (that would be full extension and referencing the photo above) and insert the valve cartridge so that its hole fully aligns with the outlet in the valve body. That should allow for the square opening in the valve arm to fit onto the square fitting in the valve cartridge properly.
