You most likely do not need a new cam. Don't sand or grind the lobe. The rocker doesn't look that bad in the pictures. I have seen *WAY* worse not just on my 4.5 but nearly every m116 and m117 I have seen in the junkyard.

Thanks Tomguy, this is good news with the cam shaft.
Anyway... I ordered a new rocker. In the picture it may seem negligible but the surface is slighly damaged/worn. I think a new rocker may help for the next couple thousand miles.

I have installed the 4 degree Woodruff key (corrects 4 degrees at crank/2 degrees at cam) at the left side cam sprocket and timing is perfect now, no retard, TDC is lining up with the cam shaft marks now.
Installing the key is easy. I didn't remove the tensioner but took tension away by installing a wood wedge between the chains at the tensioner to press the tensioner pin in. Was enough to easily remove and install the new key.
