It was a maintenance issue, not a design issue. The UA DC10 crash in Sioux City was a design issue exacerbated by a maint issue. The fan blade in the tail engine failed and disintegrated. When it broke up, it compromised the hydraulic system redundancies which were located in the tail of the aircraft near the engine that failed.
