Like phantoms, mine was loose, so...break out the trusty tools...

Let's see hammer...noo, chisel...not, screwgun...don't think so; ah trusty little screwdriver, hummm, let me see if I can, yep that works :)

I took my real small screwdriver, and at the bottom of the lighter, bent in the small ears, now they clamp to my adapter.

Also are you sure it works?, check fuse first, if the light is on, it should be working.
