Unusual transmission shifts

My 1995 E300D shifts kinda strange when I accelerate slowly. What I mean by strange is like it's hesitating to engage into the next shift. This doesn't happen when I accelerate more quickly. The car has 162,000 miles, but a Mercedes dealership replaced the transmission at 98,000 miles. For you experts out there, would this indicate my transmission in on its last leg or could there be a simpler solution like a vacuum leak, etc.?
