Won't shift into 4th

I have a 1991 300D what won't shift into 4th. the lower gears are fine it just won't shift at 40-45mph. any ideas? it has 386,000 miles.

vac valve

its the vac control on top of the fuel dist... just unhook
the vaccum and plug both and watch it start shifting..
