New bad noise....

Coming from the rear. It's kind of a pulsation as the wheel spins at certain speeds. Causes a vibration too. Not violent but certainly present. My next step was to replace the rear wheel bearings so I'm wondering if it could be a bad bearing. Possible? How would I know if my bearimng went bad. I'm hoping it's not the axle. The axles have seemed to be fine so I don't think it's spinning improperly but I don't know....Any ideas?
