Belt tensioner ML320

Serpentine belt fell off in the car wash the other day. Replaced with new belt, but alignment at tensioner pulley looks off. Did with old belt as well. Time for a new tensioner, I presume? See photos. Looks worse with engine off (second photo) compared to running (first).

Why would this happen?
