Cold/Warm Hard Start after Fuel System Work

89 190e 2.6

I just cant seem to get ahead, went to try and fix my vacuum leak and ended up with a worse car.....

Cold Start: 10 Seconds+ will eventually fire.
Warm Start: 20 Seconds, will eventually fire while pressing accelerator.
Hot Start: Fires Right Up immediately after shutting down and retrying.

Car was actually in a good spot firing right up for me cold, warm sometimes took a second turn of the key but always fired on the 2nd go, accumulator is going I'm assuming. This is worse than any hard start I've seen since I've owned the car.

Hard Start Began after Replaced/Removed:

AFM Boot
FD/AFM Oring
IACV Hoses
All Fuel Lines Removed in Process
Injectors/Seals were replaced less than a year ago.

I'm 90% sure its a fuel delivery issue, I double checked everything I touched and see nothing out of place, also not seeing any fuel leakage on any of the lines. I ran the car and let it idle for a few minutes and saw not a hint of fuel anywhere on the lines. Confirmed IACV hoses are connected and good, checked all vacuum lines nothing is out of place.

Once running car is perfect, drives just like it did before, no stumble, no hunting idle no issues at all.

A little more digging and I found a ton of fuel in the AFM boot and the intake manifold when I open the throttle body. I have no idea how its getting in there though......

Cleaned up the fuel, got everything back together. Fired up cold and then died, 2nd time fired right up. Up to temp and let it sit for 15 and no warm start.

Pulled the AFM plate and can clearly see fuel coming from the FD down into the AFM. I'm leaning toward piston oring? Can anyone comment if that sounds reasonable?
