Occasionally I feel what appears to be a shuttering effect when my 89 420 shifts from 2nd to 3rd. I am wondering if this is the beginning of the end or an easy fix. Fluid is fine, however, has not been changed for at least 30K. Thanks for help.

You need a modulator adjustment. If you have a lift available. Remove the rubber cap and pull the steel key out of the castelated top. Turn it in (clock-wise) one full turn and try it for a few days. Your shifts should be noticable firmer. One turn is a minor adjustment but if your problem is intermittant you are just on the edge and this adjustment should be enough.

Curious, why do you think it's the torque converter?

It is very rare for the torque converter to fail before the tranny on a Mercedes.
