300D Rough Starting

My '95 300D has started to miss when starting from cold. When warm, it starts smoothly. It blows considerable smoke when cold and running rough. This roughness lasts only 20 seconds and is accompanied with a louder than usual sound. Other than that the car runs great. It has 190,000 kms. on it. Any help with this problem would be greatly appreciated.
