I have to wonder about the positioning of the shut-off hook. Just like you I also took great care to install the hook in the notch and I also had a run away! Pretty scary, bet you were a little shook up. You now have joined the ranks of the few, the not so proud, and the bewildered.

To this day I have the hook on the shut-off in front of the rack, NOT IN THE NOTCH. I still have bad dreams about this.
