I have had it happen and seen it happen on other vehicles. On the ones I have experience with vibration caused the clamp to cut through the Tubing.
But, the poster said it is rusted so that could also be a cause.

I bought one a couple of years ago from the dealer, and they had to order it. I believe they are getting hard to find now. The hack is to cut out the leaking section and install a section of hose with clamps. That works, I ran mine like that for a couple of weeks until I got the part. Some folks have left that "repair" in place. I think the price was under $200 for one side.
