I got it from speedtuningusa.com. My contact there was with Oliver. I got the following mods:
-Stage 2 Programming (supposedly +60hp/+100tq)
-EGR Kill Program
-Total cost with overnight shipping was $410

It was very easy to remove/reinstall. Remove a few screws, disconnect some wires, and you can pull your entire ECU out of the engine bay. Just reverse the process for reinstallation.
