I try to be a pretty attentive driver. I habitually check my mirrors, crank the mirrors out to keep the blind spot area down, look before changing lanes, etc.

I can't begin to count the number of times a motorcycle has sneaked passed me without me even realizing he was ever there. Even with brights on, and that distinctive (loud) Harley sound, sometimes I'll have less than a half-second notice before he goes by.

It especially bothers me when we're in traffic, they're between lanes, and I'm constantly thinking about what lane has the best traffic flow.
