55-gallon, stainless steel fuel tank developed the unsourced fluke of a rusting out interior, necessitating replacement of it and the fuel pump too. Total cost: $4,800.00 or so - ouch!
ON EDIT: NOT really a mistake, but a costly mechanical product failure - several of those over the years.....
