I had the application form signed at Ken Garff Mercedes in Salt Lake City while we were there for Christmas. Pleasantly surprised to discover the nice young lady who came out into the "Ooh, that be expensive!" service input bay knew what the form was for and led me to the service manager who signed off without even looking to see what kind of car it was.
Interesting that the application form calls for 312,000 miles as the qualifying number even though 500,000 Km is 310,000+ miles. To be on the safe side, I waited until the car actually passed 312,000 before submitting the form, and included a photo of the odometer. The award took about six weeks, more or less.
