Yes, it could be a stuck injector, or a worn cam follower pin, or cam lobe or roller, it could be a loose rocker arm, a damaged crosshead, or even a worn wristpin and bushing. Its hard to say without being physically next to this engine. As for the noise coming from your ISB it is more than likely the usual clatter that emits from the VP44 injection pump used on this engine. These particular pumps have a seperate low pressure vane pump in them that supplies up to 300 psi fuel pressure to actuate the various injection timing mechanisms in the pump.
Yeah, I've been well aware of the screwy fuel supply on the 5.9L ISB's. 106k flawless miles on mine, and it also got to put up with me running homebrew biodiesel in it for a while.

I would be surprised that they would wait untill half of the injectors were down to put the engine out of service. One torching injector is enough to make very expensive repairs if it isn't fixed.

I would as a general rule check injector first as they are typically the easiest item to remove and test. I would then dig deeper if they all passed.

Then again, I'm not a Cummins repair tech (But I did sleep at a holiday in express last night!)
