Jumpy on the get go....

Today i started up my car and as usually it kicks and sputtters a bit before smoothing out (particularly in this cold 22 degree weather). But, it was on the on ramp on the highway that I had problems. As I accelerated the car started jumping back and forth in terms of acceleration. Lots of blackish brown smoke was behind me and I couldnt kick into the higher gear for a few seconds. After that incident it ran fine. What was wrong here? I know my vaccuum levers on the linkage are worn down a bit, but could it could it possibly be my injectors are a little clogged? I just changed my fuel filters. I really want to fix this before it gets real cold.
