Rough idle and smoke...

My 77' 300D doesn't idle smoothly (shakes the entire car, almost like a back and forth rocking motion) and it smokes a LOT when cold started, and smokes when accelerating from a stop. It probably smokes when underway, but it's hard to tell.

When accelerating from a stop, it seems to stutter and sometimes nails for a few seconds (almost like a misfire, or like one cylinder is down) then it smooths out and accelerates just fine. It actually has quite a bit of power. Once cruising, the engine is totally smooth and responsive.

I have replaced the fuel prefilter and there was a lot of junk in it, almost looked like black sand particles...I'm not sure where I should go from here.

Edit: I should also add that I get almost no blowby through the oil filler hole while the engine is idling...
