Odd transmission problem

Hey guys,

So I've been having a weird transmission problem basically since I've had the car. When the car is cold it shifts on time but a little hard. When it's been warmed up it shifts pretty well and goes through all the gears in order. However when it has been warm for a while or driven a long distance is when I get my problem.

The shifts get really smooth, almost flaring even, but shift 25 to 50 percent earlier than it does when it is shifting normally. Another thing is that it also likes to skip 3rd gear and go straight to 4th.

So far I have flushed the transmission twice, replaced the vacuum control valve (which I think might be a problem), adjusted said valve, and replace some vacuum connectors around the engine including transmission line connectors.

What I'm wondering is if it's a mechanical, vacuum, or fluid issue. The heat of the tranny being a factor is confusing. Fluid is the proper Castrol fluid and filters have both been made by Mann.
