What makes the car sound the way it does?

Basically, my '85 190E 2.3 8v SOUNDS old.....I don't really think it's the engine so much as the exhaust. Do engines actually emit much noise? I was think that if I added a cat back exhaust it would greatly improve the quietness/sound of the car, elimating so piston sound.
