As what Mr. jdc1244 said, check for air and vacuum leaks.
And YES, a bad injector can cuase such problem and in my experiance it would never be cleaned successfully.

The injectors start falling off after 100K..@ your mileage I'm quite certain they no longer function as they were designed.
What other service have you done in an effort to solve this issue? Jonathan
