high co readings

Hello,
looking for some advice.
changed spark plugs, oil, o2 sensor and coolant temp sensor. Readings are still off the scale. No change since changing all the items minus the coolant temp sensor, changed that this pass saturday.
Taking it in today for another shot at it. If it does not pass what else can I do to get these numbers down?
I did drive it for over 30 minutes before the test, did not shutdown the engeine before the test.
This is a 93 400e with 112,000 miles on it.
I did notice on some starts there is some shoot which comes out of the exhaust on to the driveway.
The car runs fine milage has gone down some.
Last tune up was over 6k ago.

Thanks
