Rough running returns

hi - I have a '89 260e that one year ago gave me real grief in rough running and stalling. It was repaired and had something to do with the ignition but has continually had an intermittent miss at about 2800rpm.
Yesterday, after leaving the car under a car cover in the heat for three days with very little gas/petrol in the tank, I got about 1 km from home and it started acting like a billy goat - severe missing.
This morning, I started to look at what could be wrong and when I removed the gas filler cap, there was an almighty whoosh and intake of air.

Question - would the vacuum (?) in the fuel tank been enough to cause the rough running or do I need to be looking elsewhere.

Thanks - the advice on this forum looks great - wish I had discovered it before.

sounds to me like EGR sticking
