Warm the sleeve with a propane torch.
Lube/quench with SAE30 oil, it will wick up the threads.
Apply a pipe wrench to the hex portion of the guide rod.
Use a cheater pipe on the pipe wrench as needed.

The threads are long, and will wear out your arms, wrenching it off when it is loose.
