I've seen shower heads at the end of a double cantilevered arm such as the one below. This one has the rainfall head, but, I'm sure they make one with a standard head. You can probably get the head 16" further down the tub enclosure with such a device.

It also would look reasonably good........unlike a set of hard iron pipes.
