We still have our share of accidents due to runway issues. The crash in Lexington, KY where the pilots selected the wrong runway due to construction issues comes to mind.
It's a very difficult system to run with total perfection. I find it amazing that the accident rate remains so low, considering all possible modes of catastrophe.
